Compound Informed Consent Form for Health Testing Events
Important Notice (Please Read Carefully)
Compound is a technology and information service platform and the organizer/coordinator of this event. It is not a medical institution, does not provide medical diagnosis, treatment, prescriptions, emergency care, or other medical services, and does not issue any form of medical diagnosis.
The venous blood collection and nurse blood-collection services involved in this event are organized and carried out, in accordance with the law, by Beijing Tonghe Clinic (Limited Partnership) (hereinafter "Tonghe Clinic" or the "blood-collection service provider") as an entity holding the corresponding blood-collection service qualifications, which arranges medical personnel meeting practice requirements to complete the collection at the event venue, Tower T2, Hongqiao Alibaba Center, Shanghai. Tonghe Clinic bears the corresponding legal responsibility for its blood-collection services, on-site medical safety, nursing records, blood-collection consumables, infection control, and medical waste disposal.
The testing services for this event are provided by Guangzhou KingMed Medical Laboratory Center Co., Ltd. (hereinafter "Guangzhou KingMed" or the "testing service provider"). The testing report is governed by the testing items, testing methods, reference ranges, and report date set out in the testing report actually issued by Guangzhou KingMed.
The testing results provided in this event and the health analysis report generated by Compound are for your personal health management reference only. They do not constitute medical diagnosis, treatment advice, medication guidance, or emergency services, and cannot replace an in-person consultation with a licensed physician. For any medical care, medication, treatment, or major lifestyle adjustment, you must consult a duly licensed physician.
This Consent Form involves the processing of your sensitive personal information, including identity document information, medical and health information, biological-sample-related information, and testing data. Please read it carefully and give your consent separately.
This event is intended only for adults who are at least 18 years old and have full capacity for civil conduct. Minors and persons with limited or no capacity for civil conduct may not register for or participate in this event.
If you have conditions such as abnormal coagulation function, severe needle/blood phobia (fainting), severe anemia, recent surgery, current fever or acute infection, pregnancy or lactation, or are currently taking anticoagulant medication, please proactively inform us before registration and blood collection; on-site medical personnel have the right to refuse, suspend, or terminate blood collection based on their professional judgment.
Article 1 — Description of the Parties
This health testing event involves the following parties, whose roles and responsibilities are as follows:
(1) Event Host / Platform Party
Entity name: Shanghai Compound Technology Co., Ltd. (hereinafter "we," "Compound," or the "Platform Party")
Description of responsibilities: responsible for event registration, orders and charging, event organization and coordination, user notifications, data receipt and display, generation of health information analysis reports, and platform customer service; does not provide medical services and does not give clinical direction over blood collection and testing.
(2) Blood-Collection Service Entity
Entity name: Beijing Tonghe Clinic (Limited Partnership) (hereinafter "Tonghe Clinic" or the "blood-collection service provider")
Description of responsibilities: as an entity holding the corresponding blood-collection service qualifications in accordance with the law, organizes this venous blood collection through its nurse blood-collection service, arranges medical personnel meeting practice requirements to provide on-site blood-collection services at the event venue, and is responsible for the blood-collection procedure, on-site medical safety, blood-collection consumables, infection control, medical waste disposal, on-site nursing records, and initial handling of blood-collection-related emergencies.
(3) Testing Service Provider
Entity name: Guangzhou KingMed Medical Laboratory Center Co., Ltd. (hereinafter "Guangzhou KingMed" or the "testing service provider")
Description of responsibilities: receives samples in accordance with the law, completes medical testing within the scope of its qualifications and testing items, issues testing reports, and bears the corresponding legal responsibility for the testing process, testing quality, testing results, and laboratory records.
Special note: You understand and agree that Compound does not exercise clinical direction over, or substitute its management for, the professional medical practice conduct of Tonghe Clinic and Guangzhou KingMed. Compound will perform its obligations of reasonable selection, information disclosure, organization and coordination, and assistance with communication in accordance with the cooperation agreements and the law, but does not substitute for, or assume, the medical-practice, nursing-operation, or medical-testing liability that should be borne by the blood-collection service provider or the testing service provider in accordance with the law.
Article 2 — Event Process
Registration and payment: through the Compound platform, you complete the registration information, read and sign this Informed Consent Form, the User Service Agreement, and the Personal Information Processing Rules, and complete payment at the price published by the Platform.
On-site identity verification and pre-collection inquiry: after arriving at the event venue, Tower T2, Hongqiao Alibaba Center, Shanghai, the blood-collection service provider will conduct necessary identity verification, a pre-collection health inquiry, and an assessment of suitability for blood collection.
On-site blood collection: Tonghe Clinic arranges medical personnel meeting practice requirements to perform venous blood collection for you at the event venue, Tower T2, Hongqiao Alibaba Center, Shanghai, in accordance with the blood-collection service provider's operating standards, using single-use sterile consumables.
Sample labeling, storage, and transport: after collection, the sample will be labeled, stored, handed over, and transported in accordance with the requirements of the blood-collection service provider and the testing service provider. The sample is transported to Guangzhou KingMed or its lawfully recognized sample-receiving channel.
Laboratory testing: Guangzhou KingMed completes the testing within the scope of its qualifications and testing items and generates a testing report.
Data transmission: after testing is completed, Guangzhou KingMed or its authorized system provides the testing data and/or original report to Compound.
Report display and health analysis: Compound displays the testing data to you on its website, application, mini-program, or other product pages, and generates health information analysis, lifestyle recommendations, or reports based on publicly available medical materials, a health knowledge base, algorithmic models, and rules.
The event may be postponed, changed, or canceled due to force majeure, government regulatory requirements, public health events, on-site safety reasons, or reasons attributable to the blood-collection service provider or the testing service provider. Compound will promptly notify you and handle fees already paid in accordance with the event rules and refund rules.
Article 3 — Special Note on the Nature of Services (Non-Medical-Diagnosis Statement)
3.1 Compound is not a medical institution, does not hold a Medical Institution Practice License, and does not provide diagnosis, treatment, prescriptions, emergency care, nursing operations, or other medical services.
3.2 The testing data, health scores, biological age, risk indications, lifestyle recommendations, nutrition or exercise suggestions, and the like displayed by Compound on the Platform are all health information services, not medical diagnosis, treatment plans, prescription advice, or medication guidance.
3.3 The laboratory testing report issued by Guangzhou KingMed is a record of the testing results of the relevant indicators in the sample and is not equivalent to a clinical diagnosis of any specific disease of yours. Any disease confirmation or treatment must be comprehensively judged by a licensed physician in conjunction with medical history, signs, imaging, follow-up results, and other clinical materials.
3.4 A single testing result may be affected by factors such as sampling time, diet, exercise, sleep, medication, disease status, sample quality, testing methods, and reference ranges, and there may be false positives, false negatives, borderline fluctuations, or results not fully consistent with your actual health condition.
3.5 If the testing report shows abnormalities, obvious fluctuations, or borderline values, or if you feel unwell, please promptly visit a duly qualified medical institution.
3.6 Viewing testing results and health analysis may cause anxiety, worry, or other emotional reactions. Once you view the report, the relevant information will be irreversibly known to you. If you have concerns about this, we recommend communicating with family, a physician, or a professional before viewing.
3.7 Descriptions of health, risks, indicators, and lifestyle in this event's promotion, reports, and platform content are all general health information. They do not indicate that Compound can diagnose, treat, or prevent any disease, nor do they constitute medical advertising, drug advertising, or medical device advertising.
Article 4 — Risk Disclosure for Venous Blood Collection and On-Site Screening
Venous blood collection is a routine medical-related procedure with good overall safety, but the following discomforts or risks may still occur:
Common reactions: pain at the puncture site, bruising, subcutaneous hematoma, slight bleeding, brief dizziness.
Uncommon situations: needle phobia, blood phobia, or dizziness or brief fainting caused by a vasovagal reflex.
Rare situations: infection at the puncture site, nerve injury, or allergic reaction related to disinfectant or consumables.
Persons with abnormal coagulation function or taking anticoagulant medication may experience persistent bleeding or a larger-area hematoma.
Due to factors such as vascular conditions, on-site cooperation, and physical state, multiple puncture attempts may be required, and collection may not be possible.
Due to reasons such as insufficient sample volume, labeling anomalies, sample contamination, clotting, hemolysis, breakage, abnormal storage or transport, or laboratory rejection, testing may fail or be delayed, or re-collection may be required.
You undertake that you have truthfully disclosed your health condition and any situation that may affect the safety of blood collection or sample handling, including but not limited to: a history of needle/blood phobia, coagulation disease, use of anticoagulant medication, severe anemia, current fever or acute infection, recent surgery, recent blood donation, pregnancy or lactation, a history of severe allergies, and other physical discomfort.
On-site medical personnel have the right to refuse, suspend, or terminate blood collection based on your physical condition, past medical history, medication, on-site performance, blood-collection risk, or other professional judgment. You agree to cooperate with on-site arrangements; if harm or adverse consequences result from your concealment, misreporting, or non-cooperation, you shall bear the corresponding responsibility in accordance with the law.
Article 5 — Participant Eligibility and Disclosure Obligations
This event is intended only for adults who are at least 18 years old and have full capacity for civil conduct.
Minors and persons with limited or no capacity for civil conduct may not register for or participate in this event.
You confirm that you are not an active-duty service member, a person in custody, or any other subject who, under the law, requires special approval or special protective procedures to participate in related testing or sampling activities; if you fall into such a category, please do not register or participate.
You confirm that you have no known physical condition unsuitable for participation in this event, or that you have fully disclosed it before registration and on-site blood collection.
The personal information, health questionnaire, and on-site disclosures you provide are true, accurate, and complete.
You will not impersonate another's identity, collect samples on behalf of others, or submit samples that are not your own.
Article 6 — Separate Consent for the Processing of Sensitive Personal Information
This event involves the processing of your sensitive personal information. The overall rules for the processing of personal information in this event, all of your rights, and the roles of the parties are detailed in the Personal Information Processing Rules published by Compound at the same time. This Article only discloses and obtains your separate consent for matters specific to the processing of sensitive personal information in this event.
(1) Sensitive Personal Information Processed in This Event
identity document information (such as ID number or document photo, used for real-name verification, sample matching, and report issuance);
medical and health information (past medical history, allergy history, medication history, history of needle/blood phobia, coagulation status, current fever or infection, pregnancy or lactation, and other health questionnaire information);
biological-sample-related information (the number of the blood sample collected on this occasion, collection time, and sample-related records);
testing data (testing results, reference ranges, laboratory reports issued by Guangzhou KingMed, and the health analysis data derived therefrom).
(2) Purposes and Necessity of Processing
used to verify identity, complete registration, arrange on-site blood collection, sample matching, laboratory testing, data transmission, report display, health information analysis, and customer service;
used for necessary security, quality control, complaint handling, refund handling, dispute resolution, and compliance record-keeping during and after the event;
processing or provision in accordance with laws and regulations, regulatory requirements, or the lawful requests of judicial or administrative authorities.
Processing the above sensitive personal information is necessary to complete this event; if you do not agree, we will be unable to provide this service to you.
(3) Recipients of Sharing and Entrusted Processing
Compound (Shanghai Compound Technology Co., Ltd.): as the event host, Platform Party, and personal information handler.
Beijing Tonghe Clinic (Limited Partnership): as the blood-collection service provider, processing the relevant information necessary for blood collection.
Guangzhou KingMed Medical Laboratory Center Co., Ltd.: as the testing service provider, processing the relevant information necessary for sample testing.
Necessary service suppliers (cloud services, SMS, payment, customer service, electronic signature, logistics or sample transport, etc.): processing personal information only within the minimum scope necessary to accomplish the corresponding service.
The specific scope of shared information and the roles of the parties are detailed in Article 4 of the Personal Information Processing Rules.
(4) Security Protection
The parties will adopt reasonable security measures such as encryption, access control, least privilege, log auditing, and permission isolation to protect your sensitive personal information in accordance with applicable laws and regulations and their respective management standards.
(5) Special Note on Sample Handling
Your blood sample will be used only for the testing items agreed in this event and the quality control, re-examination, and compliance record-keeping necessary to complete that testing.
Sample collection, labeling, storage, handover, transport, receipt, testing, and destruction are performed by the blood-collection service provider and Guangzhou KingMed in accordance with their respective standards and applicable legal requirements.
If the sample cannot be tested due to sample quality, sample volume, labeling, storage conditions, transport conditions, or other reasons, the relevant service provider may reject it, terminate the testing, or request re-collection.
Without your separate express consent, we will not use identifiable samples for commercial research, external sale, or uses unrelated to this service beyond the testing purpose of this event.
If, in the future, uses involving scientific research, product development, algorithm training, commercial cooperation, or other uses beyond the scope necessary for this service are involved, we will separately obtain your consent as required by laws and regulations, or use anonymized data that cannot identify a specific individual.
Article 7 — Abnormal-Result Reminders and Handling of Emergencies
At the blood-collection site, if you experience fainting, an allergic reaction, persistent bleeding, or other discomfort, the medical personnel of the blood-collection service provider will provide initial handling in accordance with their practice standards.
If the on-site situation is serious, the blood-collection service provider, Compound on-site staff, or others have the right to assist in calling the 120 emergency hotline or recommend that you visit the nearest medical institution; the medical expenses and subsequent diagnosis and treatment responsibility arising therefrom shall be handled in accordance with the law.
If the testing laboratory notifies, re-examines, or handles specific abnormal results in accordance with its internal standards or the requirements of laws and regulations, the laboratory shall do so in accordance with its rules.
After receiving the testing data, Compound may remind you, via the platform, SMS, or telephone, to view your report or pay attention to abnormal indicators, but such reminders do not constitute emergency care, diagnosis, treatment, or critical-value management services. Regardless of whether you receive a reminder from Compound, you should view your report promptly on your own; if you find any abnormality or feel unwell, you should seek medical attention immediately.
Article 8 — Withdrawal of Participation, Deletion, and Exit
Before sample collection, you may withdraw this Consent Form and exit the event; the health questionnaire and registration information already submitted will be deleted or anonymized within a reasonable period, except for information necessary for refunds, compliance record-keeping, and dispute handling.
After sample collection but before the report is issued, because the blood-collection, sample-handling, transport, and testing processes may have already begun, the blood-collection and testing processes already performed cannot, in principle, be withdrawn; however, you may still request that Compound stop displaying the report on the platform or delete the platform-side data after the report is completed.
After the report is issued, you may request that Compound delete your relevant data on the platform; however, Guangzhou KingMed, the blood-collection service provider, and others may retain the corresponding records in accordance with the law based on medical quality management, compliance record-keeping, regulatory requirements, or dispute-handling needs.
Your withdrawal of consent does not affect the lawfulness of processing already carried out before withdrawal.
Refunds or handling of fees shall be carried out in accordance with Article 6 of the User Service Agreement and the rules published on the event page.
Article 9 — Allocation of Liability
Where losses are caused to you by faults, errors, or defects in Compound's own platform technology services, event organization and coordination, order management, data display, or health information analysis services, Compound shall bear the corresponding liability within the scope provided by law.
Liability arising from the nurse blood-collection services, on-site medical safety, medical-personnel operations, blood-collection consumables, infection control, medical waste disposal, on-site nursing records, or handling of blood-collection-related emergencies provided by Tonghe Clinic shall be borne by Tonghe Clinic in accordance with the law. Compound will, within a reasonable scope, assist you in communicating with and addressing matters with the blood-collection service provider.
Liability arising from Guangzhou KingMed's sample receipt, testing process, laboratory quality control, testing report, testing-result errors, sample damage, or laboratory records shall be borne by Guangzhou KingMed in accordance with the law. Compound will, within a reasonable scope, assist you in communicating with and addressing matters with the testing service provider.
Harm or adverse consequences arising from your concealment or misreporting of your health condition, impersonation, submission of another's sample, failure to follow pre- and post-collection precautions, or failure to seek medical care in a timely manner shall be borne by you in accordance with the law.
The detailed provisions on Compound's limitation of liability are set out in Article 10 of the User Service Agreement.
Article 10 — Dispute Resolution and Governing Law
The conclusion, validity, interpretation, performance, and dispute resolution of this Consent Form are governed by the laws of the People's Republic of China (excluding the laws of the Hong Kong, Macao, and Taiwan regions).
In the event of a dispute arising from this Consent Form or this event, the parties shall first resolve it through friendly consultation.
Disputes arising from Compound platform services shall be under the jurisdiction of the people's court at the defendant's domicile, the place of contract performance, or as otherwise having jurisdiction under the law; the statutory jurisdiction rights enjoyed by consumers under the law are not limited by this clause.
For disputes involving blood-collection services or medical testing services, you may assert your rights in accordance with the law before the court of the location of the blood-collection service provider or the testing service provider, the place where the service occurred, or as otherwise having jurisdiction under the law.
Article 11 — Miscellaneous
This Consent Form takes effect from the time you sign or check to confirm it online.
The clauses of this Consent Form are independent of one another; if any clause is found to be invalid, revoked, or unenforceable, this shall not affect the validity of the other clauses.
This Consent Form, together with the User Service Agreement, Personal Information Processing Rules, Event Rules, Refund Rules, and the like signed or confirmed between you and Compound, constitutes the complete agreement. In the event of any conflict regarding matters specific to this event, this Consent Form shall prevail; for personal-information-processing matters that conflict with the Personal Information Processing Rules, whichever provision more fully protects the user's rights and interests shall prevail; for other matters that conflict with the User Service Agreement, this Consent Form shall prevail.
If this Consent Form undergoes a material change, we will inform you in a prominent location on the Compound platform and obtain your consent again where required by law.
Electronic signing, web-page checkbox confirmation, SMS verification-code confirmation, signing via an electronic signature platform, and paper signing have equal effect. Compound will retain necessary signing records such as the agreement version, signing time, account, device, IP address, and checkbox records.

Article 13 — Signature Confirmation
I have completely read and fully understood all of the above content, in particular the clauses on the nature of the services (non-medical-diagnosis), the blood-collection service entity, the testing service entity, blood-collection risks, the processing of sensitive personal information, the allocation and limitation of liability, and withdrawal and deletion. I also confirm that I have read and understood the User Service Agreement and the Personal Information Processing Rules published by Compound at the same time. I voluntarily confirm my consent in the checkboxes below:
